
First Consultation Visit
Our initial appointment will consist of consultation which includes taking x-rays i.e., periapicals, panoramics or tomographies. Dr Anjomshoaa will perform a thorough examination, review your x-rays and will explain what your diagnosis and treatment options are. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an initial evaluation and a second appointment to provide the actual treatment.
During your first visit, you can expect to receive diagnostic images, but you are welcome to bring any X-rays or scans from your general dentist or referring physician to your appointment. You may also request that your dentist or doctor forward the images to our practice for review.
Here are a few things we will ask you to provide
A list of current or prior medical conditions, including diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.
A list of the medications you are taking (heart medications, aspirin, anticoagulant therapy, etc.)
Referral forms or X-rays given to you by your dentist or physician (if any)
A valid driver’s license or photo ID
Dental insurance card Form of payment (credit card, checkbook, etc.)
IMPORTANT
All patients under the age of 18 years of age must be accompanied by a parent or legal guardian.
Please alert the office prior to surgery if you have a medical condition (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) that may be a concern or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours prior to your surgery. You will also require the company of an adult to bring you back home.
Financial Policy
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
